Patient record management systems in hospital today necessitate a competent administration when handling patients, generating reports from cashier, patient details which serves as a key factor for the flow of business transactions in Kinoni Health center. Unfortunately, the current Record management system leads to misplacement of drug details, payment details, and late release of reports and insecurity to records. This research project is aimed at computerizing all the records about patients, staff and drug suppliers. In order to achieve this goal, a thorough System Study and investigation was carried out and data was collected and analyzed about the current system using document and data flow diagrams. The concept of report production has been computerized hence, no more delay in report generation to the hospital manager. Errors made on hand held calculators are dealt out completely.

This study was carried out to design and introduce the use of an automated clinic record management system and has chosen Ahmadu Bello University Sick-bay as the case study for the first implementation with the aim of improving their services especially in this digital era. A qualitative research was adopted and the instrument used for the study are interview, observation and questionnaire where some questions were raised by the researcher such as, what are the challenges associated with the use of the current manual record management system in the Ahmadu Bello University Sick-bay? How to improve on the current manual record managements system to minimize human error in the process of the records? How to improve on the current manual time management among the staff of the clinic? And how to provide more privacy to patients’ records and information in the clinic? Fifteen (15) respondents were randomly chosen from both the clients and staff of the clinic. And at the end of the research, the findings were analyzed which led to the designing of the aforementioned system. And in order to design the programme, the researcher uses some web development tools such as; Adobe Dreamweaver and Notepad Plus Plus for the required coding; hypertext preprocessor (PHP) and Java scripts (JS) to connect the interfaces to the database; and My-SQL was used to design the databases that store the overall system data. Finally; summary, conclusion and suggestion were given on the problems associated with the manual method of record keeping such as difficulties in sorting, retrieving and updating records, lack of security of records, loss of relevant information and so on, it was also suggested that the implementation of this programme will help in improving the effectiveness and efficiency in our clinics.

The number of delays in the cause of inpatient medical records in Indonesia is still high, in the range of 45.45% - 100%. Where the highest delay in return is caused by incompleteness in filling out the medical resume, this can potentially cause losses to the hospital due to delayed claim collection. This study aims to analyze the causes of the problem to develop an inpatient medical record control system model using the waterfall model method, including Requirements, Analysis, and Design at Aqidah Hospital, Tangerang City. This case study uses a qualitative approach with in-depth interviews, FGD (Focus Group Discussion), Observation and Document Review of inpatient medical records for October – December 2021. The results showed that the delay in returning inpatient medical records in 2019 was 185 out of 280 (66.10%). Technology has provided new mechanisms and solutions to improve the service delivery provided by government agencies. One of these potential tools is an electronic records management systems (ERMS) new support tools. The high incidence of poor service delivery in government agencies is a key factor that has put pressure on the government to implement ERMS. Despite the potential benefits of implementing ERMS, the adoption and utilization of these programs has been slow and some systems have failed. Therefore, this study sought to assess how Tlokweng Land Board (TLB) implemented its electronic records management program by using Kotter's model and ADKAR change model. The study adopted a pragmatic paradigm and employed a case study research design to collect data at TLB. The research used a mixed methods approach to collect data using questionnaires, interviews and document reviews. Purposeful sampling was used to solicit data from 53 participants in the land board. The findings of the study revealed that communication has been used as the most effective tool for managing change at TLB. However, TLB has not yet managed to reinforce the change implemented due to lack of adequate training and motivation of change champions. Moreover, the change management team has little training on the change management framework produced by the Ministry of Lands and Housing (MLH). This study therefore, recommends adoption of the change management lens by records professional when implementing electronic records management systems.

Abstract/Summary
This Action Research aimed to improve the electronic records management of Accounting Department in Philippine Deposit Insurance Corporation to address the lack of having a centralized filing system of electronic files which we found out to be the cause of our Department's most common issue of having difficulty in locating electronic files and its most updated version. Addressing this underlying issue, discarded the waste of time locating a particular file and helped address other issues related to accessing, securing, and sharing of files, hence, improved the Departments efficiency and productivity.
We were able to cover two full cycles of Action Research. Different tools and methods such as 5 Whys technique, the Scheins three types of inquiry process, focus group discussion, informal conversations and one on one meeting were used to discuss the issues and identify the focal issue. The use of tools such as ORJI, journaling, and ladder of inference were also useful in doing this Action Research. All throughout the action research project, collaboration with the Departments personnel was done to come up with the best possible solution to resolve the identified issue. We adopted the use of theories related to the issue and Xiaomi Ans Best-Practice Framework of Integration for Managing Electronic Records in planning for the activities and interventions for this Action Research. In our first intervention, we agreed to prepare a User Account Maintenance Form to be forwarded to IT for the request of having a Shared Drive facility for exclusive use of Accounting Department personnel and prepared guidelines in the set-up of the Drive. As a result of the evaluation of our first intervention, we concurred to have guidelines in managing and organizing of files stored in the Drive to avoid the issues related to mismanaged files and ensure the sustainability of our change initiative and considered the idea of strengthening the back-up of the Drive which can help maximize the benefits of the Drive in our second intervention. Through the implementations of the two interventions, we are able to attain the set objectives of this Action Research and at the same time achieve other positive outcomes as discussed in the latter parts of this paper.

Health information system of St. Paul University Philippines

This research and development project aimed to examine current processes and transactions made in the health services of St. Paul University Philippines in order to gather pertinent data for the development of a proposed record management system. This proposed system aimed at improving the efficiency of the services of the clinic particularly in the creation, organization, retrieval, and updating of health information of its clientele. Ten (10) experts in Information Technology and five (5) clinic staff were the study participants. The interview with the clinic staff revealed that the existing manual system performed poorly with regards to records management in the clinic. With this, they proposed for an automated system to improve the efficiency of managing clinic records to facilitate the creation, organization, retrieval, and updating of information in the clinic transactions. Based on these results, the Health Information System for St. Paul University Philippines was developed. The proposed system was pilot tested and was evaluated by the clinic staff and IT experts in terms of its functionality. Furthermore, the participants offered suggestions to enhance the system further. From the evaluation results, it can be deduced that the proposed system is effective as this improved the recording system of the clinic.

The Web-Based Records Management for St. Paul University of Quezon City was developed in order to ease the workloads and other hassles of the doctors, nurses, and students of the school. The purpose of this research is to provide a functional Web-Based system that can assist the doctors and nurses in their daily processes, such as keeping track of patient records, monitoring inventory levels, sending SMS messages, etc. while beneftting students in terms of setting their desired appointments in accordance to their educational and personal schedule. The developers used Data Flow Diagrams (DFD’s) and Unifed Modelling Languages (UML’s) as guides in creating the system. The mobile app was built under PHP using the framework Laravel as the front-end and Microsoft Azure SQL as the back-end. The system had been evaluated by 20 end-users and 10 experts from the feld of Information Technology using the ISO 25000 questionnaires for software quality. Based on the evaluation, the system passed in software quality. The developers concluded that the desired end-users are able to utilize the system as it was meant to be used.
