Configuring Yealink DECT phone for AAISP

Relevant to: Yealink W60B, W70B, W73P, W53P and other base stations

The Yealink W60B/W70B is a DECT base station aimed at home as well as small and medium-sized businesses. Its main specifications are:

  • Up to 8 handsets
  • Up to 8 SIP accounts
  • Up to 8 simultaneous calls
  • Support for additional 'DECT repeaters'
  • IPv4 and IPv6
  • PoE or Mains plug for power
  • Transfer button (allows assisted or blind transfers to other SIP extensions/external numbers)

Credentials

The PIN is used to register handsets to the base station, and for the handset to access settings from the base station. When a Base and a handset are supplied together in a single box, then the handset should already be registered to the base station. You can use these steps to register additional handsets though:

  • Power up handset
  • Press the button under 'Reg'
  • Hold the 'Radio' button on the Base for 3 seconds
  • Handset should then initialise
  • Go to the Base station UI and set up the new account:
  • Account - Set Account2 --> Fill in details
  • Account - Handset name --> Set the name

Find the IP address of the Base station & login to setup web pages.

By default the Yealink base unit picks up an IP address using DHCP. You can find the IP address from the Handset as follows:

  • Press 'Ok' on the handset and 'Ok' on 'Status'
  • Select 'Base' and press 'Ok'
  • Under IPv4 you will see the IP address e.g. 192.168.1.3 (different for each user).
  • Enter that IP in the address bar of your Web browser.
  • Login with admin credentials. (default is user:admin password:admin).

Yealink and Voiceless Settings

On the web interface, go to:

  • Select 'Account' & 'Register' tab & enter you VoIP number (+44...) in 'Register Name' & 'Username'.
  • Enter SIP from your control page into 'Password' box.
  • Set 'Server Host' to 'voiceless.aa.net.uk'.
  • Click 'Confirm'.
  • If correct then you should see Register Status = 'Registered'.
  • If you have more than one number you can select 'Account1' drop down and add further numbers.

Repeat this for any other accounts you want to add.

Check the SIP registration status:

Handset Assignment

Here you can assign which of your (up to) 8 SIP accounts are to ring which of your (up to) 8 handsets! You can also make up to 8 calls at once, so a single base station can support a small office of people.

  • From the 'Account' tab select 'Number assignment' from menu.
  • Here you can select which handsets are connected to your numbers for outgoing and incoming calls.

You have the flexibility to decide which VoIP account should ring which handset, and also which account the handsets use to make their outgoing calls.

Transfer a call

The handsets, eg the W53H, have a dedicated 'Transfer' button 'TRAN'. This lets you transfer a call to someone else.

Assisted Transfer on a Yealink W53H handset

Say you have an incoming call that you've answered, and you want to transfer it to someone else and introdice the caller as you go...

  • Answer the incoming call, and find out who they want to be transferred to
  • At this point the caller is put on hold
  • Enter in the extension number or full number of who you want to transfer the call to
  • At this point you will be ringing the third party
  • If no answer, then you can press the button under End to go back to the original caller
  • At this point you can also Blind transfer the call, by pressing the button under Transfer
  • Once answered, you can introduce the original caller and press the button under Transfer

DND - Do not disturb

You can enable/disable DND via the handset for each of the SIP accounts allocated to the Yealink handset:

  • Hold the button on the Base for 3 seconds

Factory reset the Yealink DECT Base Station

If you have lost the password you will have to factory reset the base station, and the default username and password will then work, and you'll need to configure the unit again.

There are a few ways to factory reset the Yealink base station:

From web ui

If you know the password!

From a connected handset

You'll need to know the PIN (default is 0000)

From the actual base station

  • Unplug the power
  • Press and hold the Connect button
  • Whilst holding, plug in the power
  • Wait until all three lights on the base station are showing as solid green
  • Release the connect button
  • Wait a further 5 minutes
  • Then, reboot the base station by unplugging it from power and plugging it back in

When purchasing a Yealink W52P/W56B base unit and W52H Cordless handset(s) OR Yealink W60B/W70B base unit and W56H Cordless handset, through HostPilot or directly from a sales representative, the devices will automatically appear on your Devices list.  Most of the provisioning process is plug and play, however, you still need to assign handsets to the base in HostPilot, assign users to handsets, and register the handsets to the base unit.

Notable Limitations

  • The W52B can only   use the W52H handsets
  • The W52B is capable of 5 handsets, but can only use 4 simultaneously.

  • As with the W52B, the W56B is capable of 5 handsets, but can only use 4 simultaneously.
  • The W60B can only use the W56H handsets.
  • The W60 is capable of using 8 handsets and can use all 8 handsets simultaneously.
  • The W70B can only use the W56H handsets.
  • The W70 is capable of using 10 handsets and can use all 10 handsets simultaneously.

There are no exceptions to this rule; both bases and handsets work on different firmware versions that are dependent on their respective devices. 

Important note on Mixing Bases and Handsets

The original W52P Base supports only one type of handset; the W52H Handset. The new W60B Base supports only one type of handset; the W56H Handset.

  • You cannot use an older W52H Handset on the new W60B Base
  • You cannot use a newer W56H Handset on the old W52B Base

Set up your W52P/W56B/W60B or W70 Base unit

Powering up and provisioning..

The base unit you received has already been pre-programmed to search for the Intermedia Unite provisioning server, and your W52B/W56B/W60B or W70 base unit has already been created in your Devices table in HostPilot.  Provisioning will start immediately after the base unit is powered up and has detected an internet connection. 

  • Find a good central location for the base unit to reside; you want to make sure that all handsets get equal coverage.
  • Plug the base unit into your router.
  • Plug the power adapter into the base unit, and plug it into a power outlet.

The base will boot to begin the provisioning process. Do not unplug the base unit from the power while any of the LED lights are blinking. The base may restart several times during this process and is finished when the Power and Network LEDs are solid green. (the handset LED is only lit once handsets are registering successfully to the base)

Once the base unit lights are solid green you can start adding handsets to your base unit configuration in HostPilot.

Set up W52H/W56H Handsets

Add a handset to base in hostpilot.

In order for your handset to work with the base unit, you will need to add the handset to the base unit configuration in HostPilot. It is also necessary to register the handsets to the base.

Assigning a user to a handset allows the handset configuration to adopt information previously configured on the user.  The handset will gain the extension number and any direct dial numbers assigned to the user; this means the handset will ring any time the extension or DID receives a call. To assign a user:

  • Add the user by selecting their name in the  User drop-down field. Note: The user must be activated (with a phone number assigned) in order to assign them to a device.
  • Adjust the address information to match where the physical hardware will be located. (current hardware location will be used by default)
  • Read the 911 Policies and limitations  and click the checkbox to confirm you understand.

When the page refreshes, you will be returned to the General tab for the handset and will see User (ext number)  under  Assigned User .

Now that you have your handset added to the base and assigned a user, you should unplug the power to your base unit, wait 3 seconds, and plug it back in; this will force the base to reach out to the provisioning server and configure the line information for the handset(s).

Note: for the handsets to works, it is extremely important that you register handsets to the base in the same order they were configured to the W52P or W60B base configuration in HostPilot. 

The base organizes handsets by assigning them to Lines and each handset occupies a line between 1 and 5.  When you add a handset to the base and assign a User to the handset in HostPilot, the base keeps track of the handset by assigning that User to Line 1 for the first handset, Line 2 for the second handset, etc. This means you must register handsets with the base unit in the same order they appear on the base's Cordless Phones tab in order to ensure the correct User is displayed on the correct handset. It is good practice to mark your handsets by serial number (use the last 4 digits of each serial number of the handset) so you can properly register each handset in the correct order.

To register a handset to the base:

  • Press and hold the button on the front of the W52P/W56B/W60B or W70 base unit (about 5 secs); the registration LED will slowly flash, indicating the base station is in the registration mode
  • Press the OK button on the handset to enter the main menu
  • Go to Settings > Registration and press OK on Register Handset
  • Press OK on Base 1; the handset will search for the base registration signal Note: If the Handset has trouble searching for the base, Power Cycle the Base and try again.
  • Press the  OK  soft key when the LCD screen displays the RFPI code of the base station
  • Enter the system  PIN  (default: 9871)
  • Press the  Done  soft key.  

The handset will play a warning tone and prompt Handset Subscribed , which indicates the handset is registered successfully. After successful registration, an internal handset number and handset name appear on the LCD screen.

How do I install a VCH with my Yealink VC device?

The VCH can be installed with the VC800 and VC200 and it provides a connection between the video conferencing system and a computer. It comes with the VC800 and can be bought separately for the VC200. To setup the device it requires 3 cables. On the left side an Ethernet cable that will connect to the CP960, right-hand side’s Ethernet cable will connect to the VC800 and the HDMI cable will connect to the computer.

Yealink video conferencing endpoints VC200/VC500/VC800 support a range of bandwidth from 256 kbps to 6 Mbps. The minimum bandwidth requirement is 256 kbps to have a video conference in CIF resolution. The tables below list the bandwidth consumption for the protocols H.264HP and H.265.

Will the Yealink B2K Skype adapter work with other software?

The Yealink B2K Skype adapter can only work with Skype. It will not work with other VOIP software, such as Yate or Google Voice.

How do I make a call using a remote computer, such as Skype?

Yealink IP phones can interact with web server applications by receiving and handling an HTTP or HTTPS GET request. When receiving a GET request, the IP phone will perform the specified action and respond with a 200OK message.

Ensure that the server or computer’s IP address in in the “allow IP list”. Or you can enter “any” in “Action URI allow IP List”, which means you will allow any devices to control the phone.

To dial a number, the valid URL format is http(s):///servlet?key=number=xxx&outgoing_uri=y . XXX is the number you want to dial, and y is the SIP URL you dial from. For example, http://10.3.20.10/servlet?key=number=1234&[email protected]. It refers to account 1006 from 10.2.1.48 dial 1234 out.

What is the Jitter Buffer and how should I use it?

Jitter buffer is a shared data area where voice packets can be collected, stored, and sent to the voice processor in even intervals. Yealink provides two options for the Jitter Buffer settings, either adaptive or fixed.

A fixed jitter buffer adds a fixed delay to voice packets. You can configure the delay time for the static jitter buffer. An adaptive jitter buffer can adjust based on the delays in the network. The range of this delay time is configurable.

Maximum Delay and Minimum Delay

There are two parameters are used to limit the range of the delay. When receiving a RTP packet, it will be defined as an underflow event if the packet delay is smaller than minimum, and vice versa an overflow event if it is greater than maximum. In both cases this will cause packet loss, and lead to audio loss. The Max delay and Min delay are used only when the Jitter Buffer is configured to be “fixed”. If the Jitter Buffer is configured as adaptive, it will adjust itself accordingly base on the network latency. It is recommended to use adaptive Jitter Buffer.

Can the phone lock be scheduled for certain times?

What does the flashing LED mean?

When the phone is idle and the LED flashes, it means there is a missed call or an unread voicemail.

When the phone is in the power saving mode the LED flashes to indicate that the phone is still operating but under the power-saving mode.

How do I disable the phone lock function?

The phone lock function needs to be disabled on the Skype for Business server. Please ask your network administrator to disable it.

How do I ensure my MS account logs off when I switch off my PC?

If your PC is connected to the PC port on your Yealink phone, you can set the Yealink phone to auto log off when the PC is disconnected. In order to do this, set the following parameter on the phone: network.802_1x.proxy_eap_logoff.enable=1 If this is set to 0, then the phone will do nothing when the PC switches off.

How do I upgrade my Yealink SIP or Skype for Business device to a MS Teams edition?

First, ensure that the phone’s firmware is current or up to date. Insert the DD10K Dongle in to the USB port of the phone and reboot the phone. Set the DECT base station to registration mode. Once phone has booted up and is idle, press the Reg softkey to register phone to the base station and enter pairing password (default: 0000)

How do I apply Call Forward or DND applied individual SIP accounts?

It is possible to set call forward and Do Not Disturb (DND) to individual SIP accounts registered on the phone, rather than to all the phone as a whole. In the web interface, browse to the Features tab, set the value for Forward and DND to Custom. Now when a customer presses the DND button or sets Call Forward, they will be prompted to select which SIP account they want to apply it to.

How do upgrade the firmware on a Yealink Phone?

To upgrade firmware via web user interface:

  • Login the web interface of the phone.
  • Browse to Settings → Upgrade.
  • Click Browse and select the firmware from your local system.
  • Click Upgrade to upgrade the firmware. The browser pops up the dialog box “Firmware of the SIP Phone will be updated. It will take 5 minutes to complete. Please don’t power off!”
  • Click OK to confirm upgrade

Note: The firmware upgrade process can take a few minutes to complete. Please ensure that the device does not lose power during this process or you may cause damage to the phone.

Unfortunately, Yealink phones are not compatible with doorbell systems and thus cannot be connected to your door camera.

The firmware version has not changed after I’ve performed an upgrade

After performing a manual firmware upgrade, the firmware version has not changed.

It may be that your device is running a custom firmware version, which restricts the upgrade to a standard firmware version.

Please contact your service provide to confirm. If your phone is running a standard firmware version, please contact support for assistance.

Do Yealink IP phone support IAX?

IAX (Inter-Asterisk eXchange) is an Asterisk proprietary protocol and as such Yealink does not support this protocol.

How do I export syslogs to Syslog server?

There may be some instances where you are unable to get the syslog information from the web interface or you may require additional information for troubleshooting. In these cases, you are able to specify a dedicated Syslog server where the syslog files can be exported automatically. It is assumed that the Syslog server has been set up to store the syslog information. In the phone’s web interface browse to Settings → Configuration → Syslog section Enable to Export Syslog to server → then enter the Syslog server address, e.g. 10.2.2.10 Click Confirm to save the settings.

How do I Reset a Yealink IP Phone to Factory Default settings?

Method 1: via the web interface..

  • Log in to the web interface
  • Go to Settings → Upgrade page, select Reset to Factory Settings
  • Press OK to continue, press Cancel to quit.

Note: The reset process can take a few minutes to complete. Please ensure that the device does not lose power during this process or you may cause damage to the phone.

Method 2: Via the phone menu.

  • Press Menu Softkey.
  • Go to Settings → Advanced , enter the phone password.
  • Select Reset to Factory option. The LCD screen will show the following prompt: “Warning, Reset to Factory Setting”. Press OK to continue.

Method 3: Manual reset

Press and hold the “OK” button for about 5 seconds and the phone will automatically reset to factory default settings.

How many redundant SIP servers can be set for Yealink phones?

Yealink phones can support a maximum of 3 SIP servers for redundancy.

You are able to set SIP servers 1 and 2 via the web interface. SIP server 3 can only be set using auto-provisioning.

The LEDs on the base station do not light up

There are three LED indicators on the base station, when all three LEDs are lit solidly, this means the base station is working correctly. If not all LEDs are lit, follow these diagnostic steps: Connect a working power supply to base station, if the power LED does not light up it could be a hardware problem. Connect a network cable to the base, it will flash.

There are three cases.

  • If the network LED lights up solidly, it means that the base station has successfully connected to the network. You can register a handset to the base station, after which all LEDs should be lit solidly.
  • If the network LED does not light up at all, try a hard reset of the base station.
  • If the network LED flashes continually, it means that the base station cannot get an IP address.

Try connecting the network cable to another port on the switch, if the network LED continues to flash, try a hard reset.

If this does not resolve the issue, it may be a hardware issue.

Do the Yealink DECT handsets support Auto Answer?

The auto answer feature for Yealink DECT phones only works when the phone is placed in its charging cradle. When auto answer feature is enabled, you can simply answer the incoming calls by lifting the handset from the charging cradle To configure auto answer:

  • Press OK to enter the main menu.
  • Select Settings → Telephony → Auto Answer
  • Press Change to check or uncheck the Auto Answer checkbox.

Why is there a click sounds when I am busy on a call?

When a DECT repeater is installed in the environment, the system produces a verification tone to confirm that the handset is operational in the extended range. You can activate/deactivate the verification tone on the Repeater by following these steps:

  • Switch off the repeater (disconnect the power adapter).
  • Switch it on for 1 to 5 seconds and switch it off again.
  • Switch it on again. The LED indicator on the repeater flashes slowly.
  • Switch it off again.
  • Switch it on again.

My handset is unable to receive a call.

Check the assignment rule under Account → Number Assignment. Ensure that the extension has been assigned to the handset under the incoming call rule.

Can I Use Handsets from Other Brands with Base from Yealink DECT?

All Yealink DECT products use the CAT-iq 2.0 standard. However, to enhance the feature set of the Yealink phone, there have been some modifications to the standard.

You may be able to register other handsets to the Yealink DECT base stations, but all of the features may not be available.

You may be able to register Yealink DECT handset to other base stations, but all of the features may not be available.

It is recommended that only Yealink handsets and Yealink base stations be used together to ensure optimal performance.

How do you upgrade W52H, W53H and W56H Handset Firmware?

During the upgrade process, it is advisable to place the handset in the charging cradle and closer (within 10 meters) to the base station. While the handset is busy upgrading, you will not be able to use any of the features of the phone.

A firmware upgrade can take approximately 25 minutes. Do not cancel the upgrade or turn off the device during the process.

After completing the firmware upgrade and reboot, the handset should register automatically with the base station. If it does not register, please perform a manual re-registration.

Before upgrading W52H/W56H/W53H handset firmware, please ensure the following: The handset must be registered on the base station and within the radio coverage of the base station.

There are no external calls in progress on the base station, this includes handsets that are not about to be upgraded. The handset battery has at least 50% charge in order to finish the firmware upgrade.

Manual Upgrade

Go to the web GUI of the base station, click on Settings  Upgrade. In the Select and Upgrade Handset Firmware block, click Browse to the handset firmware file on your PC.

Click Upgrade to trigger the update. A dialog box pops up to prompt “Handset Firmware of the SIP Phone will be updated. It will take 10 minutes to complete. Please don’t power off!”.

Click OK to confirm the upgrade. The web user interface prompts “Handset Firmware updating. Please wait and do not power off. And the browser must not be closed or refreshed!”.

All handsets registered to the base station will perform the upgrade if they meet the conditions listed above. While upgrading the handset will display a progress bar with the status on the LCD screen. The handset will reboot, and the message key LED flashes red after completing the upgrade.

If a handset fails to upgrade successfully, the LCD screen will display “Upgrade fails, please upgrade again”. After a short while, the handset will attempt to perform the upgrade automatically.

How do you reset a Yealink handset to factory default setting?

Deregister handset from Base station: Press “OK” button → Navigate to “Settings” → “Registration” → “De-reg.Handset” → Enter Pin “0000” to confirm Deregistration. After specifying your handset, press “Select” → the Message “De-Register” appears Press “Yes” to delete registration from base.

Reset Handset Settings Press “OK” button → Navigate to “Settings” → “System Settings” → “Handset Reset” → Select “OK”. The Message “Reset Handset to default” appears, Press “Enter” and insert Pin code “0000”. Press “Yes” to confirm.

Yes, you can connect your mobile phone to you conference phone via Bluetooth and enable the Media audio feature to allow music to be played through your conference phone.

How do you reset a Yealink IP Phone to factory default settings?

Method 1: via web interface..

  • Login in the web UI of the phone

Method 2: Via phone menu.

  • Go to Settings → Advanced UI, enter the password to continue. Select Reset to Factory option.
  • The LCD screen will prompt you with: “Warning, Reset to Factory Setting”. Press OK to continue.

Method 3: Hard reset on the device

Press and hold the “OK” button for about 20 seconds.

Note: The phone can take a few minutes to reset to factory default settings. Ensure that the device does not lose power during this time or it may cause damage to the phone’s software.

To use the DECT connectivity of the W73H/56h/78H handsets it has to be registered with a Yealink base station. One W70B base station can handle up to ten DECT phones and handsets.

You can register a handset with four different standard (single-cell) W70B base stations if you need to move it around your office. DECT repeaters (up to six) can be used to extend the reception range of the W70B. For even greater coverage and freedom of movement, you can use the multi-cell  Yealink W80B/W90B  base stations that can manage a phone/handset across up to 60 base stations (or even up to 600 in special VIM configuration).

If you have purchased the 73P/76P/78P bundle (handset and a base station) the handset should have already been registered with the included W70B base station. If bought separately or for registering to other base stations, a manual registration is required, as explained below .

Enabling Base Station Registration Mode To register your  W73H/56h/78H  handset, your base station needs to be set to registration mode. To enable registration mode on a base station:

After you enable registration mode on the base station you can register your handset with it. 

If the handset has not yet been registered with any base station, you do an initial registration using the easy or normal methods. Otherwise, you can register it to a different base station (see next ) or switch between the ones you already registered to (see below ).

For initial handset registration do one of the following:

  • Easy Registration – the handset will search and automatically register with a base station that is in registration mode. For easy registration press the Reg soft key on the handset. The registration process will be completed automatically. If you are working with a multi-cell system (Yealink W80 or W90), the handset will register to the base station with the strongest signal in range.

If registered successfully, the screen prompts “Handset Subscribed” and “Base NO. (last 4 characters of connected Base’s MAC address).” 

After the handset is initialized, the idle screen will show the internal device number of the handset on the base station. In the example shown here the W78H is registered as "Handset 2" on the base station. The next registered DECT handset/phone will be assigned as Hanset 3.

If the registration fails it may be due to an incorrect base PIN (check with your system administrator for the correct one), the base station is out of range (move closer to it), or the base station is not in registration mode (see above .)

If you made an initial registration to a base station, the above easy/normal methods are no longer applicable. Instead, you can use the settings menu to register to another base station(s). Make sure the other base station(s) is in registration mode.

To register the handset to a different base station:

  • Select   Registration > Register Handset .
  • A list of available base stations will be shown.  Select the one you want to register to  and e nter the base station's PIN (the default PIN is 0000).
  • Press Done to complete the registration.

If registered successfully, the screen prompts “Handset Subscribed” and “Base NO. (last 4 characters of connected Base’s MAC address).”   After the handset is initialized, the idle screen will show the new internal device number of the handset on the base station. 

Switching Connection Between Base Station

If you registered a handset with more than one base station you can manually switch the connection between them or automatically select the one with the strongest signal.

To switch connection between base stations:

  • Select Registration > Select Base.
  • The screen will display all the base stations that the handset is registered with and the BestBase option
  • Select a base station you want to switch to or select BestBase to automatically pick the one with the strongest signal.

The handset is named automatically during the initial base station registration but you can rename and personalize it.

To rename the handset:

  • Select Handset Name .
  • Edit the name in the Rename   field.
  • Press the Save soft key to confirm the new name.

The handset name is also configurable via the web user interface at the path: Account > Handset Name .

Once you have registered with a base station you can rename it to make it easier to discern. Renaming may not be available on a multi-cell configuration!

To rename a base station:

  • Select  Registration > Select Base .
  • Select the base you want to rename and press Rename .   (Do not select the Bestbase option as it is used for automatically selecting the strongest signal.)
  • Enter the desired name in the Rename field and confirm the change.

There may be occasions, such as if you are replacing the base station, when you need to discard the handset registration. 

You can use any registered handset to deregister any of the other handsets on a base station. 

To deregister the handset:

  •  select Registration > De-reg. Handset .
  • Enter the base PIN (default PIN is 0000).
  • The screen will list the handsets that are registered to the selected base station. The name of the handset you are using is highlighted.
  • Select the desired handset to deregister and press OK .
  • Confirm the action when prompted.

You can also deregister a handset via the web user interface at the path: Status > Handset&VoIP > Delete Handset .

Locating a Handset

If you are using the W70B single-cell base station you can locate a misplaced registered handset/phone by paging it from the base station.

To locate a handset:

  • All the handsets that are registered with the base station will ring (get paged) simultaneously and display the IP address of the base station.

You can also locate handsets via the web user interface at the path Status > Handset&VoIP > Paging Status .

Before Dr. Bobby Mukkamala — an ear, nose, and throat specialist in Michigan — prescribed postsurgical opioids recently, he checked state records of his patient’s existing controlled substance prescriptions, as legally required. A score generated by a proprietary algorithm appeared on his screen. Known as NarxCare (now used by most state prescription monitoring databases, major hospitals and pharmacy chains), the algorithm indicated his patient had an elevated risk of developing an addiction to opioid painkillers.

“I create a lot of pain when I operate,” said Dr. Mukkamala, who leads the American Medical Association’s Substance Use and Pain Task Force. “The nose and the face are very painful places to have procedures done.” Consequently, it is difficult to avoid prescribing opioids to manage pain.

Algorithms like NarxCare and a newly approved genetic test for opioid use disorder risk known as AvertD use machine learning techniques to try to help doctors reduce the odds that patients will become addicted to these medications.

Via NarxCare, most Americans now have an opaque equivalent of a controlled substance credit score, which they often don’t even know exists unless a doctor or pharmacist tells them that it’s a problem. (NarxCare’s manufacturer claims that its scores and reports “are intended to aid, not replace, medical decision making.”) And if it ever becomes widely used, AvertD, promoted as a way to use personalized genetics to assess risk, could put yet more difficult-to-challenge red flags on people’s records.

These tools may be well intentioned. But addiction prediction and prevention is a mind-bogglingly difficult task. Only a minority of people who take opioids become addicted, and risk factors vary for biological, psychological, sociological and economic reasons.

Even accurate scores can do harm, since addiction is stigmatized and often criminalized. Some people have been expelled from physicians’ practices for having high NarxCare scores, with no way of appealing the decision. Others were denied postsurgical opioids by nurses or turned away from multiple pharmacies, with little recourse.

These kinds of algorithms could potentially worsen race and class biases in medical decision making. It’s not hard to imagine a dystopian future of unaccountable algorithms that render some people forever ineligible for pain care with controlled substances.

Dr. Mukkamala noted that closer scrutiny of his recent patient’s medical history showed there really wasn’t reason for concern. “What’s inappropriate is for me to look at any number other than zero and say: ‘Boy, this person’s got a problem. I can’t prescribe them anything for their pain,’” he said. Many medical professionals, however, don’t have his level of knowledge and confidence. Prejudice against people with addiction is common, as is fear of being charged with overprescribing — and the algorithms’ scores only feed into those concerns. Different, also unaccountable, algorithms monitor physicians’ prescribing patterns and compare them with their colleagues’, so this is not an overblown concern.

When I reported on NarxCare in 2021 for Wired , I heard from patients who were left in agony. One said that she had her opioids stopped in the hospital and was then dismissed from care by her gynecologist during treatment for painful endometriosis, because of a high score. She didn’t have a drug problem; her score seems to have been elevated because prescriptions for her two medically needy rescue dogs were recorded under her name, making it appear she was doctor shopping. Another high-scoring patient had his addiction treatment medication prescription repeatedly rejected by pharmacies, even though such medications are the only treatment proven to reduce overdose risk.

More recent research and reporting confirm that scientists’ concerns about the widespread use of the software remain and that patients are still reporting encountering problems because of potentially incorrect risk assessments and medical staff members’ fears about disregarding NarxCare scores.

To generate risk scores, NarxCare apparently uses variables like the number of doctors someone sees, the pharmacies they visit and the prescriptions they get and compares an individual’s data with information on patterns of behavior associated with doctor shopping and other indicators of possible addiction.

But there is no transparency: The NarxCare algorithm is proprietary, and its information sources, training data and risk variables — and how they are weighted — aren’t public.

Another problem for NarxCare is that opioid addiction is actually quite uncommon — affecting 2 to 4 percent of the adult and adolescent population, despite the fact that a 2016 study shows some 70 percent of adults have been exposed to medical opioids. “Identifying somebody’s base line risk of opioid use disorder is inherently going to be pretty difficult,” said Angela Kilby, an economist who studied algorithms like NarxCare when she was an assistant professor at Northeastern University. “It’s sort of like trying to find a needle in a haystack.” The rarity of the condition possibly lowers the algorithm’s precision, meaning that most positive tests may be falsely positive simply because the base line rate of the disorder is low.

Research shows that about 20 percent of the time, people who are flagged as doctor shoppers by identifying risk factors similar to those apparently included in NarxCare in fact have cancer: They typically see multiple specialists, often at academic medicine centers where there may be teams of doctors writing prescriptions. The algorithm can’t necessarily distinguish between coordinated care and doctor shopping.

Likewise, people who are visiting multiple doctors or pharmacies and traveling long distances might be drug seeking, or they could be chronically ill and unable to find care locally. Some states also put information from criminal records into prescription monitoring databases, and this can lead to bias against Black and Hispanic people simply because racial discrimination means that they are more likely to have been arrested.

There’s also a more fundamental problem. As Dr. Kilby notes, the algorithm is designed to predict elevations in someone’s lifetime risk of opioid addiction, not whether a new prescription will change that trajectory. For example, if someone is already addicted, a new prescription doesn’t change that, and denying one can increase overdose death risk if the person turns to street drugs.

Recently, NarxCare has been joined in the addiction prediction game by AvertD, a genetic test for risk of opioid use disorder for patients who may be prescribed such medications, which the Food and Drug Administration approved last December. Research by the manufacturer, Solvd Health, shows that a patient who will develop opioid addiction is 18 times as likely to receive a positive result as a patient who will not develop it. The test, which looks for specific genes associated with motivational pathways in the brain that are affected by addiction, utilizes an algorithm trained on data from over 7,000 people, including some with opioid use disorder.

But that F.D.A. approval came, surprisingly, after the agency’s advisory committee for the test voted overwhelmingly against it. While the F.D.A. worked with the company behind the test to modify it based on the committee’s feedback, it has continued to raise concerns. And recently a group of 31 experts and scientists wrote to the F.D.A. urging it to reverse course and rescind its approval. Some of the group’s concerns echo the problems with NarxCare and its algorithm.

For a study published in 2021, Dr. Alexander S. Hatoum, a research assistant professor of psychological and brain sciences at Washington University in St. Louis, and his colleagues independently evaluated the algorithm elements used for a tool like AvertD, based on information published by the company. They found that all the iterations they tested were confounded by population stratification — a problem that affects genetic tests because they reflect the history of human ancestry and how it changed over time because of migration patterns.

When AvertD was being considered for F.D.A. approval, Dr. Hatoum and his colleagues wrote a public comment to the agency that said genomic variants used in the test were “highly confounded by genetic ancestry” and did not predict risk any better than chance when population stratification is not taken into account. (At a 2022 meeting, Solvd’s chief executive claimed AvertD adjusted adequately for population stratification; the F.D.A. did not reply directly to a question about this claim.)

Dr. Hatoum’s work also demonstrated that these tests could mislabel people who are descended from two or more groups that were historically isolated from each other as being at risk of addiction. Since most African Americans have such admixed ancestry, this could bias the test into identifying them as high risk.

“This means that the model can use the genetic markers of African American status to predict opioid use disorder, instead of using any biologically plausible genetic markers,” said D. Marzyeh Ghassemi, a professor at M.I.T. who studies machine learning in health care.

In an email, Solvd said that in its clinical study of AvertD, “no differences in performance were seen by race, ethnicity or gender,” adding that it was undertaking postmarketing tests as required by the F.D.A. to further evaluate the test. The company also critiqued Dr. Hatoum’s methodology, saying that his study “asserts a false premise.”

The F.D.A. said in a statement that it “recognizes that in premarket decision making for devices, there generally exists some uncertainty around benefits and risks,” adding that it had nevertheless “determined that there is a reasonable assurance of AvertD’s safety and effectiveness.”

Still, the agency has placed a black box warning on AvertD, forbidding its use in chronic pain patients and emphasizing that the test cannot be used without patient consent. But this is unlikely to be a genuinely free choice: Patients may fear being stigmatized as potentially addicted if they don’t agree to be tested. And false negatives that incorrectly label someone as low risk may conversely lead to careless prescribing.

Amid the opioid crisis, it is understandable that regulators want to enable technologies that could reduce risk of addiction. But they must ensure that such algorithms and devices are transparent as to their methods and limitations and that they reduce racial and other biases rather than reinforce them.

Lakers vs. Nuggets Game 2: The three biggest mistakes Darvin Ham made during stunning second-half collapse

It took a number of mistakes for the lakers to lose to the nuggets on tuesday.

The Los Angeles Lakers had a very real chance to not only steal a game against the Denver Nuggets on Monday, but swing the entire series. They led by 20 in the third quarter. They are 28-14 at home, and a win would have given them home-court advantage in the series. A year ago, the Lakers won two series as underdogs because they swiped one of the first two games on the road and never lost at home. Would you have bet against LeBron James in a best-of-five series with three home games?

Instead, well, you've seen the final score by now. The Lakers blew a 20-point lead . They lost to Jamal Murray at the buzzer. Now, they're down 2-0 in the series. They've lost 10 in a row to the Nuggets as a whole. The daunting prospect of still needing to win a game in Denver looms over the rest of the series. The Lakers will get only two more tries. In NBA history, 449 teams have gained a 2-0 series lead. Only 33 of them have come back to win the series.

And yet, in many ways, the game felt familiar. The recent history of the Lakers-Nuggets rivalry has revolved around three close quarters followed by a crunch time bloodbath. Entering Monday, the Lakers and Nuggets had played a total of 18 clutch minutes since the beginning of the 2023 Western Conference Finals. Denver won those minutes by a whopping 32 points. In Game 2, the Nuggets won the final five minutes and eight seconds by 10 points. Time and time again, once the Nuggets decide they are ready to take the Lakers seriously, they blow them out of the water. Head coach Darvin Ham never knows what levers to pull in response.

It takes dozens of things going wrong to blow a 20-point third-quarter lead. You need bad shooting variance. You need a bad whistle. You need sloppy, isolated missteps that are the natural outcome of tense, playoff basketball games. But more than anything, you need strategic mistakes, and the Lakers made several of them. Here are the three biggest ones they made down the stretch to allow the Nuggets the chance they needed to complete the comeback:

The rotation

Spencer Dinwiddie played 13 minutes in Game 1. The Lakers lost those minutes by 12 points. Though his size carries defensive upside, he's lost too much of a step to defend in a series like this properly. Teams frequently leave him open from deep and dare him to shoot. His 3-point numbers in Los Angeles have generally been good. For his career, he's a 33.3% shooter from deep. All of that is more or less how his second-half minutes played out on Monday. He entered the game with the Lakers up 10. Roughly four minutes later, he left with the Lakers up five. The Nuggets didn't guard him. They didn't notice him on defense either.

Ham isn't exactly in an easy position here. The Lakers signed Gabe Vincent for exactly this series. They watched him defend Murray well in the Finals. He's made his shots in high-pressure playoff moments before. But Vincent missed most of the season. As a result, he never quite earned Ham's trust in the way the front office likely hoped. So he played just 15 minutes in this one. He wasn't great, and didn't make a shot. But he's at least a threat from deep and his defense this postseason has largely been good. Disperse those Dinwiddie minutes to Vincent and Austin Reaves , who played only 33 in Game 2, and the outcome is potentially different.

And then there's the higher stakes matter of when to play LeBron. James took a seat with 3:44 remaining in the third quarter for his typical second half rest. But Ham allowed him only one minute of reprieve before re-inserting him into the game. This did two things that would eventually come back to bite the Lakers. The first is that it left James visibly exhausted for the most important minutes of the game: the beginning of the fourth quarter, when Nikola Jokic traditionally takes his rest. The Nuggets lost 25 games all season... but only 14 of those losses saw Denver outscored during Jokic's minutes. In the other 11, Denver won the Jokic minutes but lost the game because of how badly their bench lost its own minutes. The key to beating Denver is winning these minutes. The Lakers lost the no-Jokic minutes by a point because James was gassed and Anthony Davis was out.

To address James' exhaustion, Ham ultimately removed him again when Jokic returned. Again, his absence was for less than two minutes, and the Nuggets made up four more points while he was out. Yes, having James on the court at the end of the third quarter helped, but it deprived the Lakers of their best chance of knocking the Nuggets out for good. If a rested James takes the court early in the fourth-quarter, their lead might have grown enough to be safe.

Going away from their best play

Look at how easy this looks...

The TNT broadcast told us when the second half began that James and Davis stayed on the court during halftime to work on their pick-and-roll. When the third quarter started, the Lakers proceeded to run that play, their best play, every single time down the court. 

For five years, this has been their finishing move, their late-game checkmate to put a helpless opponent away. The Nuggets tried several coverages to slow it down. Jokic played a high-drop on the first video in that clip, so James snuck a pocket pass between the two defenders for the and-one. They blitzed the second, so James passed above them, giving Davis yet another easy layup over a too-small low-man (in this case, Murray). Jokic dropped lower on the third, and Murray shuffled over to help, so James nailed Reaves for an open 3.

Properly spaced, this is one of the best plays in basketball. There's no good answer to it unless you have two seven-footers that can switch (good luck with that). Go over the screen and James beats you to the rim. Go under, and now that he's evolved into a lethal shooter, he buries you from deep. Defend at the level of the screen and you're begging for a lob. Drop and you give James a head of steam to potentially draw a foul. Bring in a third defender and one of the best defenders in NBA history finds his shooter. It's an unguardable weapon that the Lakers are comfortable spamming when the need arises. Behind this play, the Lakers took their 15-point halftime lead up to 20.

And then? They... just... stopped... running it. Not fully, but they didn't run it anywhere close to often enough. James started using Rui Hachimura as his dance partner, often stashing Davis, the worst shooter in the starting lineup, in the strong side corner, where help could more easily arrive. D'Angelo Russell became a more frequent ball-handler as well, but his limited explosiveness gives him weaknesses that James doesn't have, especially against a Denver team that wins through discipline and intelligence rather than pure athletic skill.

Are there explanations for why the Lakers made this call? Sure. James and Davis were both exhausted by the end of the game. Maybe they lacked the stamina to abuse their best play as aggressively as they would have liked. The Nuggets also adjusted by switching that play more aggressively, so that when James got his screen, he ran into Aaron Gordon instead of Jokic. Ham had no response to this adjustment.

But in truth, it's a play the Lakers don't use often enough under any circumstances. For whatever reason, the Darvin Ham Lakers tend to get disorganized on offense and revert to isolation-ball far more often than the Frank Vogel versions did. The Lakers got sloppy down the stretch offensively when they'd found a play that consistently gave them easy points.

Not doubling Jokic

Before Game 2, Ham made an admission about defending Jokic that most coaches would secretly agree with. "It's like shit, I don't know what to do," he joked . To an extent, he's right. Jokic doesn't have a weakness. There's not a specific coverage that's going to stifle him. You're choosing between bad answers. It's just that Ham chose the worst answer down the stretch: he allowed Jokic to play one-on-one in the post. 

Jokic scored three times in the final five minutes. Davis attempted to guard him one-on-one each time. Jokic shot just below 53% when guarded by Davis in last year's Western Conference Finals. It's a matchup he's completely comfortable in. He even managed to get Davis airborne on the final shot he took of the night, picking up an and-one for his troubles.

Ham did have an adjustment here, but not a successful one. On Denver's final two possessions of the game, the Lakers put James on Murray and switched his pick-and-roll with Jokic. Once Murray realized this, he took advantage to set up a quick jumper before the switch set-in that tied the score at 99. And then, on the final possession of the game, the Nuggets took the switch to get Davis on Murray on the perimeter. They cleared the strong side so Murray would be able to work one-on-one. He created just enough space over the exhausted Davis to drill the game-winner.

Was there necessarily a good answer to any of this? No. But it's worth noting that Nuggets not named Porter or Jokic did not make a single 3-pointer in Game 2. They obviously could've heated up if given the chance, but given the circumstances, some help on Denver's two best players likely would have been warranted. It's just easier to stomach losing on role-player 3-pointers when those shots haven't been falling all night.

Every Lakers player and coach bears some responsibility for the loss, but strategic mistakes are the single fastest way to get knocked out of the postseason. Simply put, Michael Malone out-coached Ham on Monday, and that robbed the Lakers of their best chance to win this series.
