In fact, we gave it an A- for this area, which is one of our highest grades. We weigh several aspects of a nursing home's inspection report in computing our inspection scores. One key factor is deficiencies. It should be noted that the severity of the deficiencies is usually more important than the number of deficiencies, as some of these end up being relatively insignificant. This place received 3 deficiencies on its inspection report, but we were relieved to see that none were considered to be severe deficiencies. This indicates that the inspectors did not deem any of the deficiencies to be an immediate risk to resident health or safety. Remember that deficiency-free inspection reports are uncommon in the industry.

Short-term Care Quality

This facility was awarded an impressive short-term care grade as well. We gave it one of our better grades in that category with a grade of B. In forming our short-term care grades, we look at the facility's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. The purpose is to formulate a tool for sizing up the rehabilitation services of various facilities. Fortunately, it appear that this nursing home has registered nurses on staff. Not all facilities employs these skilled professionals. However, based on the information they provided, it does not look like the facility employs physical therapists. The last statistic we looked at in this category is the number of residents who were able to leave the facility and return home. We discovered that 4.6 percent of this nursing home's residents returned home.

I went with my sister to visit my uncle tested negitive for covid 19 before I went had good time with uncle but yesterday my sister called me tested positive tested myself positive i have atrial fibulation cancer survivor can not afford to get covid you people need to get your act together

For the second time within the past year and a half my sister was an inpatient needing rehab. Let me just say what dedicated, kind, patient, and capable folks she was taken care of by. Significant changes have occurred and I was very impressed with their commitment. Nursing, PT, OT, CNA, Social Worker/discharge planning. I hope I have not forgotten any one as I want to thank everyone. Thank you for your care and commitment.

This place is awful , my mother was placed here and whenever I would visit the staff was lazy and took there time to check the well being of the patient. They acted like it was a chore . It is so hard to get in touch with anyone for no one answers the phone EVER! They are secretive when you ask them a question in regards to your person being there like they cant tell you anything . The attitude of the nurses suck . My mother passed away here as well and when i came to clean her room out the nurses couldn't tell me how she passed leading me to believe it was suspicious , her injurious didn't go with the story the report gave. One day she was fine the next day she was dead . And im saying this because with me being there and seeing the way she was treated makes me believe she died from their neglect . They need more staff who can speak English properly and understand more. This place sucks and I was to young to care for my mom myself which is the only reason she was here in the first place me being only 16 at the time and not in her custody . I am 23 now and very able to advocate and i advise no one to leave there loved ones here !

DO NOT PUT YOUR LOVED ONES IN THE NURSING HOME!!!!!! The nurses AND doctors are INCOMPETENT and LAZY. My relative was there for less than 3 weeks and he is now in ICU because of an INFECTED PRESSURE ULCER!!! He didn't have any pressure ulcers when he arrived at Presentation and it took them 2 weeks to notice that he had an ulcer and that it was infected!!! This place should be SHUT DOWN!!

This place is terrible, Definitely the worst place my mother ever stayed. I wish there was a way to make them pay for the horrible things she endured while she was there, but I am afraid of retaliation happening to other elders I know who might have to go there, or to me myself if I ever have to go there, so I have kept quiet. But if you are reading these reviews and trying to find a place for your loved one PLEASE DO NOT send them to this place unless your intention is for them to deteriorate quickly. I still think about it every time I ride thru Oak Square in a car or on the bus. When I am driving I avoid going down that part of Washington Street so I won't even have to see the sign for Presentation Road--just passing that intersection makes my gut clench with horrible horrible memories. i WISH THERE WERE A WAY TO GIVE ZERO STARS PLEASE DON'T SEND ANYONE HERE IF YOU CARE ABOUT THEM AT ALL

Victor M. Mukhin was born in 1946 in the town of Orsk, Russia. In 1970 he graduated the Technological Institute in Leningrad. Victor M. Mukhin was directed to work to the scientific-industrial organization "Neorganika" (Elektrostal, Moscow region) where he is working during 47 years, at present as the head of the laboratory of carbon sorbents.     Victor M. Mukhin defended a Ph. D. thesis and a doctoral thesis at the Mendeleev University of Chemical Technology of Russia (in 1979 and 1997 accordingly). Professor of Mendeleev University of Chemical Technology of Russia. Scientific interests: production, investigation and application of active carbons, technological and ecological carbon-adsorptive processes, environmental protection, production of ecologically clean food.
